Who can get referred?
Dementia Buckinghamshire supports all people with a diagnosis of dementia who do not live in residential care and are either living in Buckinghamshire or are registered with a GP practice in Buckinghamshire.
We accept referrals from the health care sector and voluntary sector, as well as self-referrals from people with a diagnosis, their family and carers.
How to make a referral
